ВЛИЯНИЕ ДИНАМИЧЕСКОЙ ФИКСАЦИИ ПОЯСНИЧНЫХ ПОЗВОНОЧНЫХ СЕГМЕНТОВ НА ИХ ПОДВИЖНОСТЬ

Аннотация

Цель исследования. Сравнительная оценка влияния динамических имплантатов COFLEX, DIAM и DYNESYS на подвижность поясничных позвоночных сегментов in vitro . Материал и методы. Работа выполнена на 15 не подвергшихся заморозке препаратах позвоночника человека (по пять сегментов L2—L3, L3—L4 и L4—L5). Мышечные ткани удаляли, но связочный аппарат, дугоотростчатые суставы и межпозвонковый диск оставляли интактными. Для изучения подвижности сегмента использовали устройство с тензометрическими датчиками, позволяющее регистрировать прикладываемую силу и линейные перемещения позвонка по трем взаимоперпендикулярным осям. Определяли диапазон линейных флексионно-экстензионных и боковых движений позвонков между нагрузками -120 и +120 N, а также величину нейтральной зоны. Измерения проводили на интактных сегментах после частичной резекции элементов заднего опорного комплекса и после установки динамических имплантатов. Результаты. Установка имплантатов COFLEX привела к уменьшению диапазона линейных флексионно-экстензионных движений на 57 %, DIAM — на 32 % и DYNESYS — на 69 %. Все три имплантата уменьшали объем флексионных движений в сагиттальной плоскости соответственно на 70, 57 и 87 %. Линейную экстензионную подвижность существенно снижали имплантаты COFLEX и DYNESYS и незначительно — DIAM. При боковых наклонах существенное уменьшение диапазона движений на 25 % было выявлено только при применении системы DYNESYS. Заключение. Межостистые имплантаты COFLEX и DIAM ограничивают движения позвонков только в сагиттальной плоскости, тогда как транспедикулярная система DYNESYS уменьшает подвижность позвоночного сегмента и во фронтальной плоскости.
