НЕЙРОГЕННЫЕ ДЕФОРМАЦИИ ПОЗВОНОЧНИКА ВЗРОСЛЫХ: СОВРЕМЕННЫЕ ПРОБЛЕМЫ И ПОДХОДЫ К ЛЕЧЕНИЮ

Аннотация

Цель исследования. На основании данных литературы показать особенности клинических проявлений, диагностические аспекты и подходы к лечению нейрогенных деформаций позвоночника взрослых.

Материал и методы. В обзоре литературы использовали базы данных PubMed, Medline, Web of Science, Scopus, CrossRef, Clinical Key, eLibrary, ресурс AO Spine и библиографию ключевых статей с 6 февраля по 4 ноября 2017 г.

Результаты. Определены общие тенденции в лечении взрослых больных с нейрогенными деформациями позвоночника. Показано, что необходима разработка протоколов ведения данных пациентов с определением основных клинических симптомов, обоснованием использования неинвазивных, малоинвазивных или других вариантов помощи. Хирургическое лечение больных с деформациями позвоночника при нейродегенеративных (болезни Паркинсона) заболеваниях сопровождается высоким уровнем осложнений и повторных вмешательств. Тактические алгоритмы для этих пациентов должны включать этапность нехирургических и мини-инвазивных методик, а также клинические проявления мио-, миело-, радикулопатии, которые в сопоставлении с диагностическими тестами могут определять показания и объем декомпрессивных вмешательств, протяженность и уровни фиксации позвоночника.

Заключение. Многокомпонентность патогенетических механизмов, неоднозначность результатов нехирургического и хирургического лечения определяют необходимость мультидисплинарного подхода и разработки протоколов ведения взрослых пациентов с нейрогенными деформациями позвоночника.
