БОЛЕЗНИ ДОНОРСКОЙ ЗОНЫ КАК ПРОБЛЕМА ХИРУРГИЧЕСКОЙ ВЕРТЕБРОЛОГИИ: СИСТЕМАТИЧЕСКИЙ ОБЗОР

Аннотация

Наиболее часто применяемым пластическим материалом в хирургической вертебрологии является костный аутотрансплантат из гребня подвздошной кости. Несмотря на высокую эффективность аутогенной костной ткани при спондилодезе, ее использование связано с рядом отрицательных моментов, в том числе с так называемыми болезнями донорской зоны, то есть осложнениями забора костного аутотрансплантата. Большие осложнения связаны с техникой забора трансплантата. Из малых осложнений наибольшее значение имеет формирование в отдаленном послеоперационном периоде хронических болевых синдромов, частота которых составляет 28-31 %. Хронические боли в донорской зоне могут быть связаны с мобилизацией мышц, разрушением наружного кортикального слоя крыла подвздошной кости, повреждением нервов. Лечение хронической боли в донорской зоне остается практически исключительно симптоматическим. Обобщены основные факторы, влияющие на частоту осложнений при заборе трансплантата из гребня подвздошной кости, и технические приемы, способствующие ее снижению.
