ХИРУРГИЧЕСКОЕ ЛЕЧЕНИЕ ПЕРЕЛОМОВ ШЕЙНОГО ОТДЕЛА ПОЗВОНОЧНИКА ПРИ АНКИЛОЗИРУЮЩЕМ СПОНДИЛИТЕ

Аннотация

Цель исследования. Анализ результатов хирургического лечения пациентов с переломами шейного отдела позвоночника при анкилозирующем спондилите.

Материал и методы. Пролечены 6 пациентов с переломами шейного отдела позвоночника при анкилозирующем спондилите. У всех пациентов были чрездисковые переломы, полученные при низкоэнергетической травме, чаще всего на уровнях С5–С6 и С6–С7. Все пациенты страдали как минимум одним сопутствующим заболеванием, спинной мозг был поврежден в 100 % случаев.

Результаты.В послеоперационном периоде у всех пациентов зафиксированы осложнения. При анализе послеоперационных рентгенограмм установлено, что деформация позвоночника большей частью корригирована, на уровне оперативного вмешательства достигнута стабильная фиксация позвоночника. У трех пациентов, которым проведено оперативное вмешательство на передних отделах, отмечен регресс неврологической симптоматики. Больные из группы А по Frankel умерли без какихлибо признаков регресса неврологической симптоматики на 2–7е сут после операции от сердечнолегочной недостаточности. При аутопсии выявлено тяжелое повреждение спинного мозга. Летальность составила 50 %.

Заключение. Хирургическое лечение переломов шейного отдела позвоночника при анкилозирующем спондилите в условиях стабильной фиксации уменьшает риск осложнений и позволяет провести раннюю активизацию пациента.
