Хирургия болезни Шейерманна. Основные проблемы: несистематический обзор литературы (часть I)

Аннотация

Цель исследования. Многофакторное сравнение двух хирургических стратегий в лечении пациентов с тяжелыми формами болезни Шейерманна.

Материал и методы. Поиск источников (Scopus, Web of Science) позволил обнаружить 56 публикаций, содержащих искомую информацию. Анализ литературного массива проведен по четырем направлениям: результаты одно- и двухэтапных вмешательств сравниваются по степени достигнутой коррекции и ее сохранения, количеству осложнений, продолжительности операции и объе-му интраоперационной кровопотери, уровню качества жизни пациентов в послеоперационном периоде.

Результаты. Объем достигнутой коррекции кифотической деформации и послеоперационной потери коррекции у больных после одно- и двухэтапных операций практически идентичны. Осложнения, связанные с имплантатами, несколько чаще отмечены после одноэтапных операций, гнойные, неврологические и прочие осложнения – после двухэтапных. Продолжительность операции и объем интраоперационной кровопотери выше при двухэтапных операциях. Качество жизни, оцененное по результатам применения различных опросников, весьма существенно улучшается, независимо от типа оперативного вмешательства.

Заключение. Двухэтапная оперативная коррекция кифозов Шейерманна не имеет заметных преимуществ перед одноэтапной операцией, однако необходимы новые исследования с большими (более 15–18 лет) сроками послеоперационного наблюдения.
